Understanding Severity Ratings:
RED - AVOID COMBINATION: Significant risk of harm. Requires immediate physician discussion. Do not combine without explicit medical oversight.
AMBER - USE WITH CAUTION: Meaningful interaction but potentially manageable with dose adjustment, timing, or monitoring. Physician and pharmacist should be aware.
GREEN - NO SIGNIFICANT INTERACTION: Combination appears safe based on available evidence. Still practice caution and inform your healthcare team.
IMPORTANT: This guide covers the most common and clinically significant interactions found in the Vanguard supplement library. It is not exhaustive. Drug-supplement interactions are influenced by genetics, doses, timing, food, age, kidney/liver function, and individual factors. Always consult your physician and pharmacist before starting or stopping any supplement.
2. Cardiovascular Medications
2.1 Anticoagulants (Warfarin, DOACs)
Anticoagulants prevent blood clots. Many supplements can increase bleeding risk or reduce anticoagulant effect. This is one of the highest-risk medication-supplement interaction categories.
KEY POINT: Warfarin (Coumadin) requires frequent INR (International Normalized Ratio) monitoring. Any new supplement warrants discussion with your anticoagulation clinic. Direct Oral Anticoagulants (DOACs) like apixaban and rivaroxaban have fewer dietary interactions than warfarin, but the above supplements should still be discussed with your provider. IMPORTANT DOAC NOTE: Berberine is a strong CYP3A4 inhibitor. Since apixaban and rivaroxaban are CYP3A4 substrates, berberine can raise DOAC blood levels and increase bleeding risk. If you are on a DOAC and considering berberine, discuss with your anticoagulation team. Rating: AMBER. Monitor for bleeding signs.
2.2 Statins (Atorvastatin, Rosuvastatin, Simvastatin)
Statins lower cholesterol by inhibiting HMG-CoA reductase. Several supplements can interfere with statin metabolism or have overlapping effects. Some interactions increase risk of muscle damage (rhabdomyolysis).
NOTE: Rosuvastatin is metabolized via CYP2C9 rather than CYP3A4, making it safer with CYP3A4 inhibitors like berberine. However, check with your pharmacist about your specific statin. **CoQ10 depletion by statins is well-documented; supplementation is recommended, not restricted.
2.3 Blood Pressure Medications
Blood pressure medications work via different mechanisms (ACE inhibitors, ARBs, beta-blockers, calcium channel blockers, diuretics). Several supplements can have additive BP-lowering effects or interfere with electrolyte balance.
CRITICAL: Potassium supplementation with ACE inhibitors or potassium-sparing diuretics is a medical emergency risk. If you are on either class of medication and considering potassium supplementation, your doctor MUST order a baseline serum potassium level and monitor it. Licorice (found in some herbal formulas and licorice root supplements) actively works AGAINST blood pressure medication.
2.4 Antiplatelet Agents (Aspirin, Clopidogrel)
Antiplatelet medications prevent dangerous blood clots but increase bleeding risk. All blood-thinning supplements are additive and should be discussed with your cardiologist.
These interactions are especially important post-stent or post-MI. Dual antiplatelet therapy (aspirin + clopidogrel) is standard after certain cardiac events, making additional blood-thinning supplements riskier.
3. Diabetes & Metabolic Medications
3.1 Metformin
Metformin is the first-line diabetes medication. It works via AMPK activation and hepatic glucose reduction. Several supplements share similar glucose-lowering mechanisms, creating additive hypoglycemia risk.
**Metformin frequently causes B12 depletion over time. Supplementation is recommended for long-term users, especially those >50 years old. Alcohol significantly increases lactic acidosis risk—the most serious metformin side effect.
3.2 SGLT2 Inhibitors (Empagliflozin, Dapagliflozin)
SGLT2 inhibitors work by blocking glucose reabsorption in the kidneys. They have unique safety concerns related to volume depletion and euglycemic DKA.
IMPORTANT: SGLT2 inhibitors can rarely cause euglycemic diabetic ketoacidosis (DKA), a serious condition where blood is acidic but glucose is not extremely high. This is harder to recognize than typical DKA. Ketogenic diets increase this risk.
3.3 Insulin & Sulfonylureas
Insulin and sulfonylureas directly lower blood glucose. These medications carry the HIGHEST hypoglycemia risk when combined with glucose-lowering supplements.
CRITICAL: Insulin patients must be especially cautious about glucose-lowering supplements. Hypoglycemia (low blood sugar) can cause seizures, coma, or death. If you are on insulin, do NOT add ANY glucose-lowering supplement without explicit physician approval and frequent glucose monitoring.
3.4 GLP-1 Agonists (Semaglutide, Tirzepatide)
GLP-1 agonists are newer diabetes medications also used for weight loss. They work by slowing gastric emptying and increasing satiety. This creates unique absorption interactions.
GLP-1 medications are generally well-tolerated with supplements. The main concern is delayed pill absorption. Timing is key: take other oral medications well before or well after your GLP-1 injection.
4. Thyroid Medications
4.1 Levothyroxine (Synthroid, Others)
Levothyroxine replacement has MORE absorption interactions than virtually any other medication. It requires careful timing and coordination with supplements. The narrow therapeutic window means improper absorption leads to hypothyroidism or hyperthyroidism.
TIMING RULE: Take levothyroxine on an empty stomach, first thing in the morning. Do NOT take calcium, iron, magnesium, or zinc supplements within 4 hours. Wait 30-60 minutes before eating or drinking coffee. **Biotin is particularly sneaky—it doesn't affect thyroid function but inflates TSH on standard lab tests, potentially causing inappropriate dose changes.
4.2 Liothyronine (T3) & Desiccated Thyroid
T3 has a shorter half-life and narrower therapeutic window than T4. Absorption interactions are MORE clinically significant.
T3 supplementation has a narrower margin of safety. The above absorption interactions are even more critical with T3 than T4.
5. Psychiatric & Neurological Medications
5.1 SSRIs & SNRIs
Serotonin reuptake inhibitors work by increasing serotonin. Several supplements increase serotonin independently, creating risk of serotonin syndrome—a potentially dangerous condition. This is one of the highest-risk supplement interactions.
Serotonin syndrome signs include agitation, confusion, rapid heart rate, high blood pressure, dilated pupils, muscle rigidity, and fever. It is a medical emergency. If on an SSRI, avoid methylene blue, St. John's Wort, 5-HTP, and tryptophan absolutely. **Fish oil (especially EPA-enriched formulations) actually has evidence supporting its use alongside SSRIs for depression.
5.2 Benzodiazepines & Sleep Medications
Benzodiazepines enhance GABA, the brain's primary inhibitory neurotransmitter. Any supplement that increases GABA or CNS depression is potentially dangerous.
The most dangerous combination is benzodiazepines + alcohol or kava. This can cause life-threatening respiratory depression. Benzodiazepines are involved in significant overdose deaths, especially when combined with CNS depressants.
5.3 Monoamine Oxidase Inhibitors (MAOIs)
MAOIs (phenelzine, tranylcypromine) are rare but require extreme caution. Nearly EVERYTHING interacts with MAOIs.
CRITICAL MESSAGE: If you are on an MAOI, do not add ANY supplement without explicit pharmacist review. MAOIs have dangerous interactions with food, supplements, and other drugs. Your pharmacist must be your gatekeeper.
5.4 Modafinil & Armodafinil
Modafinil is a wakefulness-promoting agent. It induces CYP3A4, which metabolizes many drugs.
6. Hormonal Medications
6.1 Testosterone Replacement Therapy (TRT)
TRT increases testosterone and DHT. Several supplements affect DHT conversion or have hormonal effects.
Supplements that lower DHT (like saw palmetto) may reduce the virilization benefits of TRT. Your endocrinologist should know if you're using DHT-lowering supplements.
6.2 Finasteride & Dutasteride
6.3 Oral Contraceptives
Birth control pills are estrogen/progestin combinations. Several supplements reduce their effectiveness.
Women on birth control who are taking St. John's Wort or modafinil have significantly increased unintended pregnancy risk. Backup contraception is necessary.
7. Pain & Anti-Inflammatory Medications
7.1 NSAIDs (Ibuprofen, Naproxen, Prescription NSAIDs)
NSAIDs inhibit COX enzymes and increase bleeding risk. Several supplements have additive antiplatelet or GI risk.
NSAIDs are among the most misused medications. Chronic NSAID use damages the GI tract and kidneys. Lithium patients are at particular risk—even low-dose lithium orotate supplements can reach toxic levels when taken with NSAIDs.
7.2 Acetaminophen (Tylenol)
**NAC (N-acetylcysteine) is the actual pharmaceutical antidote for acetaminophen overdose. Supplemental NAC may provide liver protection. Combining chronic acetaminophen use with alcohol is a major cause of liver failure.
8. Proton Pump Inhibitors (PPIs)
PPIs reduce stomach acid. While this helps GERD, it impairs absorption of several nutrients and medications. Long-term PPI use creates multiple micronutrient deficiencies.
IMPORTANT: Long-term PPI use is associated with vitamin B12 deficiency, magnesium depletion, and increased fracture risk. Many patients on long-term PPIs are not told that supplementation is recommended. If you've been on a PPI for >6 months, discuss B12, magnesium, and calcium supplementation with your doctor.
9. Cancer Therapies (Brief Overview)
Cancer treatment is highly individualized. Supplement interactions with cancer therapies are highly context-dependent. This section highlights critical principles only. Any patient on cancer treatment MUST consult their oncologist before supplements.
Critical Principles:
Antioxidants may interfere with therapy: Some chemotherapy and radiation work by creating oxidative damage to cancer cells. High-dose antioxidants (vitamins C, E, NAC, curcumin, resveratrol, green tea) may reduce treatment efficacy. NOT all chemo relies on oxidative stress, so context matters.
CYP induction interactions: St. John's Wort induces CYP450 enzymes, reducing levels of many chemotherapy agents. AVOID absolutely.
Immunotherapy + immune supplements: Immunotherapies activate the immune system to attack cancer. Immune-stimulating compounds (medicinal mushrooms, high-dose vitamin D, astragalus, thymosin) may cause immune dysregulation or worsen side effects.
Drug-herb interactions are severe: Chemotherapy drugs have narrow therapeutic windows. Interactions can dramatically reduce efficacy or increase toxicity.
If you have cancer, your oncologist must approve EVERY supplement.
